The Escalator (Janet Cunningham) Janet drops her keys on a mall escalator. As she bends to pick them up, her scarf gets caught in the side comb-plate. The escalator’s maintenance panel is missing, and a loose bolt causes the steps to misalign. She is pulled into the mechanism, and the escalator essentially scalps her.

Soon after, the survivors begin dying in elaborate, “accidental” ways that follow the order of the vision’s intended deaths. Death’s design follows the order of death in Nick’s vision (not the order they leave the track). Here is the complete kill sequence: final destination 4 full

The drain cover is loose due to a previous repair error. When the pump activates, the suction force pulls her entire body against the grate, eviscerating her internal organs through a small opening. Nick panics and forces his group to leave. Seconds after they exit, the vision comes true exactly as he saw it. They survive, along with a few others: a security guard (George Lanter), a racist redneck (Carter Daniels), a mother with her young son (Samantha & Tim), and two mechanics (Andy & Nadia).
